If you do not wish us to have such personal information then you should not complete the Contact Form. Please note that you will be asked to provide the same personal information if you call our toll free number instead of completing the online contact form. Our primary purpose in collecting and processing personal information is to provide you with the information you have requested regarding the genetic studies that we are conducting on anorexia nervosa and the possibility of participating in such studies. Depending on your location, we will direct your contact information to a participating research site to provide you with the information you have requested regarding how you can participate in the genetic studies. If you express an interest in participating in the genetic studies, you (or your parent or legal guardian, if you are under the age of 18) will be required to sign a written consent form that describes the study and your participation in it prior to being permitted to participate in the study. If you decide not to participate in the study or if you (or your parent or legal guardian) refuse to give your (or their) written consent, or if it is determined that you are not eligible for participation in the studies, personal information collected from you shall not be retained and shall be deleted from our records in the normal course.
