Paying for Prescription Medications  

CHADD Medical Benefit Program  Discount Prescription/Vision & Eye Care Card 

Children and Adults with Attention-Deficit/Hyperactivity Disorder (CHADD) is a national non-profit organization serving
individuals with ADHD and their families.  CHADD has developed a discount card to help defray the cost of prescription
medications and vision and eye care expenses.  One discount card covers both prescription and vision & eye care for members

Discount Prescription
Save up to 60% on most prescriptions.  Prescriptions are available through U.S. mail order pharmacy or use the card at over
40,000 pharmacies throughout the U.S. (Wal-Mart, Kroger, Osco, Albertsons, K-Mart, Medicine Shoppe, Publix, Rite Aid,
Target, Winn Dixie, and more). 

Discount Vision & Eye Care
Save up to 60% when using the discount card at thousands of independent providers and participating JC Penney Optical, Sears
Optical, Target Optical, or Pearle Visions.  Save on contact lenses, frames, lenses, and more. 

Eligibility Requirements
For eligibility requirements for both CHADD members and non-members, contact the CHADD representative office at
501-851-8556 or call the CHADD hotline at 1-800-233-4050. 

Cost
One discount card covers the entire household.  CHADD Member = $30.95 per year (this fee covers the entire household).
Non-Member = $49.95 per year (this fee covers the entire household)

To have a registration form sent to you in the mail, call CHADD’s hotline at 1-800-233-4050.

Medical Assistance (Medicaid) recipients should call the Pennsylvania Medicaid Program to find out which prescriptions are
