After you have received your letter from SWAN saying that it is time to schedule an annual appointment you do so either by phone or by email. If you decide to schedule by email it is important that you give your name, address, phone number, as well as dates and times you are available. Once processed you will either receive a conformation email or notice that your dates and times were unavailable.
